How it runs, in four phases:

Seed. Name the course, the exam, the date, and what it covers.
Sprout. Write ten questions you think will be on it, from memory, looking at nothing.
Grow. Answer all ten cold, then open the book and mark each one Got it, Half, or Nothing.
Harvest. Rank everything you missed by points at stake and commit to the top three tonight.
